In a cardboard wallet which originally doubled as a mailing envelope; it was originally available as a special offer with the Queen Elizabeth CD - purchasers had to return an enclosed sticker with a first-class stamp and the disc would be sent to them using the card cover as envelope, the address and stamp being put on the blank fold-over flap. Remaining copies were later available from Head Heritage mail-order with un-adulterated flaps!
